The student named above has my permission to attend Sing Out, 2023. I hereby agree to indemnify and hold harmless Opera Theatre of Saint Louis and its employees from and against any and all claims of personal injuries of any kind arising from participation in Sing Out. Further, I authorize Opera Theatre of Saint Louis staff and faculty to seek emergency medical help for my child if this becomes necessary. I realize that every effort will be made by Opera Theatre of Saint Louis staff to contact me in the event of a medical emergency, and I agree to indemnify and hold harmless Opera Theatre of Saint Louis personnel in seeking medical care for my child.

PHOTO AND VIDEO CONSENT: By my signature, I agree that Opera Theatre of Saint Louis may use photographs or video featuring the student (named above) in the routine promotion of its classes and activities and for other non-commercial applications.

FIELD TRIP CONSENT: I give permission for my child to leave the premises of the camp for field trips related to Opera Explorers camp. I understand that these trips are supervised by one or more adult staff members. I also understand that I will be fully informed as to the day, time and location of the field trip.

Opera Theatre of Saint Louis reserves the right to dismiss any camper who does not comply with camp rules.
